Defect Description :
Quiz answer sheet does not reset after taking the quiz again
Steps to reproduce the issue :
Go to main page.
Go to 'Computer science and Engineering' under 'Broad areas of virtual labs'.
Go to "Soil Mechanics Lab".
Click "list of experiments".
Click any experiment
Go to Quiz.
Answer the quiz and click 'grade me'
click take the quiz again.
Expected result :
Empty answer sheet to be displayed which will be answered
Actual Result
The previously answered quiz answer sheet is displayed
