PCF on vSphere with NSX-V : Monitoring Port place holder is missing in param file

Expected result

As the param file, we suppose to have place holder for Port and Monitoring Port for NSX-V -- Load Balancer -- Pools where as the param file only have place holder for one port which is not clear as that port representation which field.

Steps to Reproduce

https://github.com/pivotal-cf/pcf-pipelines/blob/master/install-pcf/vsphere/params.yml -- Line number 221 ,216 , 211, 226. Currently we have only one port where the NSX-V required 2 ports one for Service Port and one for Monitoring Port.

Possible Fix

Add place holder for the missing port.
