iTerm2 color scheme does not match Xresources

The iTerm2 color preset provided does not match the Xresources color scheme. I recreated the Xresources one in iTerm2, available here.

I've manually added iTerm2 colours a while ago when switching OS: https://github.com/w0ng/dotfiles/blob/master/iterm2/hybrid.itermcolors
Will update README and comments with updated link.
