Theme Header and Background changes are not being logged

Some themes, such as Twenty Eleven, have added support for Custom Headers and Custom Backgrounds in the Appearance menu.
Currently, Stream is not tracking these changes at all. They should be added to the Settings connector with the contexts being Custom Header and Custom Background, respectively.
This is related to #308 since these settings can also be changed via the Customizer.
