Unable to add question to quiz

I'm not able to add questions to the quiz. Suppose I'm adding question 10 then after filling in all the question and answer details when I click on the Save button, the page shows Question added. But when I click on Back to Overview total number of questions is 9 only for that quiz.

I tried the Database repair option available in Global Settings but it's not helping out anyway.

Please suggest what could be the fix for this issue.
