Table query wizard gives limited records (Most records which were modified lately)

What steps will reproduce the problem?
1. Table to Query - Accounts
2. Fields to include - First Name, Last Name
3. No clauses or filter criterias selected

What is the expected output? What do you see instead?
Expected: All records with First Name & Last Name in the system
Actual output: Few records show up, mostly the ones that I visited or modified 
manually through SFDC

What version of the product are you using? On what operating system?  With
what Salesforce.com edition?
Version 16.3
Windows 7
Salesforce - Enterprise Edition

Please provide any additional information below.
I am new to this but i successfully installed the excel addin and managed to 
extract all Birthdates once and updated necessary changes. Since then I have 
not been able to extract all records.
