adding up enrollment risk HHS module

@yubin-park - In the HHSHCC module, the enrollment risk is being added up for all 11 months for each risk profile. Instead, it should probably be entered as a parameter in the profile. Following up on my email.

@yubin-park - i created a couple pull requests that address this issue and use enrollment months as a parameter for the HHS HCC package. let me know what kind of testing you would like to see before you merge them to the main branch. thanks.
