Form fields not passing in post

The information is not passing in the post only the values that I specify "is value" are going through.

I tell this to just about everyone -- you haven't mapped the fields correctly. Look at the debug email to see what the form provides, then copy those field names/indexes (without the square brackets) to the source mapping.
