replace jotform for signups (include "how did you hear about us", waiver, orientation, automatic welcome email, etc)
implement cancellation with an "exit interview"

Created member registration and added "how did you hear" with auto welcome email. ac66cb0

Added a "why left" option. Figure that admins can manually take care of this.
