Special PR template for staging -> master PRs

As we are discussing some of the nitty gritty for how staging -> master PRs should work, I think its clear that these PRs are quite a different beast than what our current PR template prescribes.
As we determine what kinds of checks and considerations need to be made for a staging -> master PR, we should add that to this new PR template.
Some things I think should be included as of now:
What overall changes are being included in this "make it live" PR?
What staging commits/PR #'s are included in this "make it live" PR?
Did we do a last check with the affected html files using html preview?
