Objective

To make it easier for care givers to follow up on medium and long term action items for care receivers

Method

A care giver can log in and see receivers in their team's database. They can select an existing care receiver or add a new one and assign a care plan to that person. Once assigned, a care giver will receive reminders to complete action items to care for that particular care receiver. They can mark them as complete or uncompleted, or postpone them.
