To get people started with CC in a Box, it was recommended to have a /start page with our current google form embedded into the page rather than in a separate window. It doesn't have to look nice, just work.

Hey @ndesai06 and @RajChaudhry, I've created the form and placed it in communityconnect/public/ as suggestions.html . Let me know if you have any issues!
