Caves - some times after Mario falls through the trap floors he get instantly teleported far away and out of bounds.

The image shows the crazy position he is.

Possible issue when the platform gets removed from mario feet. It only happens some times and there's no particular setup to force it to happen.

Same can happen in City of Vilcabamba in the block room where you get the initial key.
