DICOM-to-NIfTI fails when specifying a directory with a DICOMDIR and DICOM files

In this case, the DICOMDIR is searched for SeriesInstanceUID at top-level. The get_files function should look for DICOMDIRs even in directories, and merge duplicate filenames in the list.
