SciGraph returning incorrect text/indices on Monarch API

When I query /api/nlp/annotate/entities, I get back incorrect text values (see full description at monarch-initiative/scigraph-docker#11). This might be a reversion of the SciGraph indices bug (SciGraph/SciGraph#281).
