Exercise - Hide solutions does nothing

The Hide Solution link does nothing in the editor. It should be removed. Users can see the solutions folded in Preview.
This was a UX decision based on watching lots of authors concern over this.
Providing "hide solution" completely solved that problem. It would be
better to fix whatever is interfering the hide solution than to remove it.
