Update link to code of conduct

Hi there!
Poking around for possible pull requests, I noticed that the readme.md has an outdated link pointing to code of conduct.
The link assumes that the code of conduct is stored in a CONDUCT.md, when in fact it's stored in CODE_OF_CONDUCT.md.
Same holds true for the issue and pull request templates.
I fully understand that this is trivial and I will submit a pull request to amend this myself in the next minute or so, but I thought it was worthing pointing out as a newcomer.
