header not printing

The header appears online in the pdf, but doesn't print.

I'm not sure if this is a margin problem. If so, could you help me to reset all margins to 1"?

You're in the US ? Try to set the paper format to Letter, It's A4, the European default, now.

You set the paper format on the Read Offline->PDF settings page, it's the first setting.

I change the paper format to Letter. Still the same clipping of the header when I print from the browser window (Chrome) in preview:

. I have found that if I open the pdf in Adobe Pro, it prints correctly.

Nice to know that it works fine in Adobe Pro. I've experienced the similar in Chrome, so I print mine using Preview on Mac OS X.
