Create Organization - Form changes (Part 2)

NOTE: To be picked up after #2206 is completed and merged.
When logged in as a site admin, the user can create new organizations in allReady.
Having reviewed the UX of this form we should add a visual clue (probably an asterisk) to required fields. Currently a user cannot identify these fields until they submit a form with any incomplete, required fields.
The following fields are currently considered required and should be marked with a red asterisk:
- Name/Organization Name
- Privacy Policy URL or Privacy Policy (Either one of these can be completed, but must be one and only one!)
- Address1/Address Line 1
- City
- State
- Postal Code
- Country
We should include a message "Fields marked with an * are required" message at the top of the form.
