Thanks for this. I am working on getting this to work in my shopify site. So far it's worked great. I have been able to create a contact us page and the receive it at my proper email address.

Where i run into problems is at step 10 - Part 2, making it look good.

On my shopify site I am not sure where to insert the javascript into my index.html - I can edit the code on my shopify theme, but i don't have an index.html file that I can find yet.
